Job Summary:
 Implement and maintain an Antimicrobial Stewardship Program at Children's
hospital and provide support to other network hospitals caring for children.
 Perform antimicrobial pharmacokinetics consults.
 Work collaboratively with other clinicians and healthcare providers to
implement and maintain innovative disease management programs and
clinical pharmacy services to optimize the pharmacotherapeutic aspects of
patient care. Execute such improvements via direct patient care, committee
participation and clinical coordination of pharmacy staff.
 Apply principles of Pharmaceutical Care to practice including assurance of
the correct drug, dose, and route of administration; recognition of un-addressed
patient needs, recognition and resolution of pharmacotherapeutic problems.
 Document all clinical interventions and other information as required by law
and departmental policies and procedures.
 Participate in the professional development and competency of all
pharmacist staff. Function as a role model and an educational resource for
pharmacy staff. Provide drug information and training to all healthcare
personnel, including physicians, nurses, pharmacists, and pharmacy
technicians.
 Execute safe and appropriate pharmaceutical care for patients by developing
and utilizing evidence-based and best-practice protocols. Develop and
implement policies and procedures as it relates to specialty. Promote and
support safe medication management.
 Share responsibility for network and departmental publications, educational
programs and pharmacy initiatives to promote pharmacy as a respected source
of drug therapy information and as a disease management resource.
Consistently contribute to pharmacy publications and execute educational
programs and pharmacy initiatives.
 Support the residency programs and serves as a preceptor for the pharmacy
residents (PGY1 and PGY2) and Pharm.D. Students.

Five years of work experience. A Pharmacy Practice Residency can substitute for
three years of the required experience. A Specialty Residency can substitute for
two years of the required experience.

Requirements:
 Pharm.D
 Completion of an ASHP-accredited residency and specialty training is
preferred
 Experience and a general understanding of pharmacokinetics and
pharmacodynamics in critically ill patients are preferred
 Board certifications within specialty is preferred
 Previous experience as a pharmacy residency or student preceptor is
preferred
 Five years of work experience. A Pharmacy Practice Residency can substitute
for three years of the required experience. A Specialty Residency can substitute
for two years of the required experience.
