SUMMARY:
The Home Health Physical Therapist is responsible for the evaluation and
treatment of patients with orthopedic, neurological, complex-medical and
other diagnoses that affect a person’s strength, range of motion, endurance
and overall function. The Home Health Physical Therapist will utilize
therapeutic exercise, modalities and activities to achieve the highest level of
function possible
The Home Health Physical Therapist is accountable first, to the Home Health
and Hospice Clinical Nurse Manager and secondly, to the Director of Home
Health, Hospice and Community Outreach
Responsible for an initial evaluation, including objective goals, of patients
referred for Physical Therapy prior to beginning treatment. Treatment will
occur only with a written referral of a health care provider with approved
privileges to prescribe Physical Therapy services
This is 32 hour a week job also this is a 64-hour per pay period position.

$2,000 RELOCATION
PAY STARTING A $50 PER HOUR
Mileage reimbursement is .50 per mile
QUALIFICATIONS:
Must be a graduate of an American Physical Therapy Association
accredited program and hold minimally a Bachelor’s degree, preferably a
Master’s Degree in Physical Therapy;
Must have at least 2 years experience
Willing to locate to Colebrook NH;
Competent in Computer use: Windows
BLS certification required.
