Manages care management team of nurses and social workers in a case
management, discharge planning, utilization management and social services
division.

Our client is a unique, community hospital in a breathtakingly picturesque rural
setting with easy coastal access and easy access to Seattle. This progressive
employer seeks a Manager of Care Management / social services to join a
tight-knit, cohesive staff, while working with well-regarded leadership. This
forward thinking facility boasts a music therapy program and offers a good many
preventative care and community education programs.

Responsible for daily operations of the department including:  provision of
ongoing discharge planning activities, ongoing social work intervention for
in-patients and out-patients, and coordination and provision of UM activities and
functions.  Must be able to work effectively with patients/families of all ages and
cultural backgrounds.  Works closely with medical staff members and patient
care staff of the organization to ensure appropriate patient outcomes and
resource utilization.  The ideal candidate will possess working knowledge of
factors impacting hospital clinical practice, including industry trends,
reimbursement issues, standards of care, appropriate utilization of various
levels/settings for health care, and treatment modalities.  Oversee 7 FTEs and
report to the CFO.

This individual is decisive, a risk-taker, collaborative, open-minded,
team-focused, and will possess an understanding of community based
healthcare in a rural setting. Department oversees discharge, arranges for other
levels of care (e.g., assisted living), coordinates transportation, and assists in
obtaining medical equipment.  Department helps with public assistance,
insurance requirements, community resources, support groups, and outside
agencies for counseling.

Requires: RN. BSN preferred. Certification and/or membership in professional
clinical outcomes management organization(s) preferred.  Current professional
licensure in Washington State required (or eligible). A minimum of five years as
a registered nurse in a discharge planning, utilization management, social
services and care management arena, with minimum of one year experience in
field of utilization/case management. Minimum of three years of leadership
and/or management experience in similar setting preferred.
